Team Elite Sports Academy (TESA) General Liability Waiver & Release Form

This waiver and release of liability is entered into between Team Elite Sports Academy (TESA) (“Facility”) and the undersigned participant (and parent/guardian if participant is under 18).

Acknowledgment of Risks

I acknowledge that participation in athletic training, sports activities, gym classes, or the use of equipment at Team Elite Sports Academy (TESA) involves inherent risks, including but not limited to:

• Risk of physical injury (sprains, fractures, muscle strains, etc.)

• Risk of property damage or loss

• Risk of exposure to contagious illness

I understand and voluntarily assume all risks associated with participation at TESA.

Release of Liability

In consideration of being allowed to participate at TESA, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby:

• Release, discharge, and hold harmless Team Elite Sports Academy (TESA), its owners, directors, staff, coaches, volunteers, and partners from any and all claims, liabilities, or demands for damages resulting from participation, except in cases of gross negligence or intentional misconduct.

• Acknowledge that this release applies to all activities at the Facility, including classes, open gym use, practices, training sessions, and events.

Medical Consent

In the event of an emergency, I authorize TESA staff to seek medical care for me/my child and accept financial responsibility for any medical treatment.

Facility Rules & Expectations

I agree to comply with all posted rules, staff instructions, and safety guidelines. I understand that failure to follow TESA rules may result in suspension or removal without refund.

Minor Participants (Under 18)

If the participant is under 18 years of age:
I, the parent/legal guardian, consent to my child’s participation at TESA and agree to all terms of this waiver on their behalf.
